Lost Passport
To get a duplicate, you have to fill out (Would you please type) the attached forms and send them to the Consulate, along with the following documents:
- A police record stating the loss of your passport.
- Consular Card. (if you do not have one you have to register, please refer to Consular registration).
- The original of your Carte d'Identite Nationale.
- A photocopy of the passport you are declaring lost.
- A photocopy of the Alien Registration card or equivalent.
- 4 passport pictures (Would you please write your full name & your Carte Nationale # on the back of the pictures).
- Fee $100 (Please write your name & address on the money order).
Please note the following:
- NO PERSONAL CHECKS ARE ACCEPTED as a form of payment.
- Please note that the passport renewal & extension time process is 10 business days. The period may be shorter or longer depending on the amount of files received. Please call us only if you do not receive your passport within 10 business days at: 212-758-2625.
- Please use express mail.
- Fees for returning your passport are not included in the fees above. To ensure the reception of your passport, please include a prepaid express mail envelope obtained from the post office. Please note that this is the only mailing method accepted. Thank you.
