Marital Status Certificate Requirements
- A duly completed and signed
Fiche Individuelle des Renseignements
- Copies of the Moroccan ID (Carte Nationale) and passport of the applicant

Copy of the birth certificate of the applicant

Affidavit form completed by the applicant and signed before a notary public
- Two Witness Affidavit forms completed by two male witnesses and signed before a notary public
- A copy of the Moroccan passports as well as the Moroccan IDs of the afore-mentioned witnesses
- A Money Order in the mount of $18 US (additional $9 for registration/carte consulaire + 2 photos)
- A prepaid Express Mail return envelope
